Oregon’s “Return to Play” program is a comprehensive initiative designed to ensure the safe and gradual return of young athletes to their respective sports following injuries, particularly concussions. The program emphasizes a step-by-step approach, where medical professionals, including chiropractors and sports therapists, closely monitor each athlete’s recovery, assessing their readiness through various physical and cognitive tests. 

The Return to Play protocol prioritizes the health and long-term welfare of the athletes, recognizing the importance of fully healing before resuming competitive play.

In Oregon, students in grades 6 to 12 are required to have a physical examination once every two years.

In Oregon, students in grades seven to 12 are required to have a physical examination once every two years if they want to start a new sport or begin a new competitive season.

Some schools require a new sports physical when entering 9th grade regardless whether the current sports physical is valid. Check with your high school to see if a new one is required for Freshman.

  • Test your vision (bring glasses or wear contacts, if needed)
  • Measure your height and weight
  • Check your heart, lungs, belly, and ears
  • Check your joints, strength, flexibility, posture, and reflexes
  • All student-athletes must have a parent/guardian signature on the form.
  • Minors under 15 must be accompanied by a parent/guardian.
  • Minors over 15 may come alone if they have a parent/guardian’s signature.
